TNR Release Form - I acknowledge that:

1) Craven/Pamlico TNR Project provides spay/neuter for community/stray cats & I certify to the best of my knowledge the cat(s) that I am admitting for spay/neuter are not homed.

2) I hereby authorize Craven/Pamlico TNR Project & its volunteers, Craven Pamlico Animal Shelter & its staff, as well as other animal hospitals we may use, to perform necessary surgical/medical procedures to ensure my cat(s) will not reproduce. I understand that these procedures will be performed to the best abilities of staff & volunteers and that outcomes are not guaranteed. I further understand that cats face risks during trapping, handling, anesthesia & surgery & I hold Craven/Pamlico TNR Project & its volunteers, Craven Pamlico Animal Shelter & its staff, as well as any other animal hospitals we may use, harmless should a cat experience complications, injury, escape or death. Any cat deemed by the veterinarian to be severely ill or injured will be humanely euthanized.

3) In addition to sterilization, these cats will be ear tipped to identify them as being sterile, free roaming cats. Routine preventative health care (vaccines, parasite control, etc.) will be performed according to the veterinarian's recommendations & availability of resources.

4) I promise these cats will be safely sheltered & fed after surgery & that I will follow recovery instructions provided at the time of discharge.

5) I will return all cats to the location from which they were taken, following guidelines by Craven/Pamlico TNR Project & agree that no cat will be surrendered to a shelter or relocated once presented to us for sterilization.

6) I agree to return to pick up the listed cats at the specified time. Any cats not picked up will be considered abandoned & a report of illegal abandonment will be filed.

7) I agree to allow Craven/Pamlico TNR Project to use photos or videos of the cats for promotional or educational purposes.
